Core Responsibilities & Expectations for the Role Participant Care
- Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.
- Exhibit and honor Habitat's Values.
- Provides occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
- Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
- Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers.
- Makes home visits as needed and documents activities including participant/caregiver response in participant's chart.
- Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME.
- Documents therapy services provided and performs department record-keeping within documentation requirements
- Continue to raise the bar. Constructively seek and share feedback and help us implement changes to improve clinical outcomes and experience for participants.
- Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
- Maintains the quality assurance, safety, environmental and infection control programs.
- Performs data collection, prepares reports and maintains any necessary participant statistics.
- Attends all staff meetings and patient care conferences as scheduled.
- Masters or Doctorate in Occupational Therapy (MOT or DOT) or equivalent from Accredited Program meeting AOTA & ACOTE standards
- National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
- A minimum of one year experience working with the frail or elderly.

- 2+ years as a practicing OT.
- Bilingual Spanish/English or Cantonese/English preferred, other language abilities are also a plus
- PACE experience and/ or ADCH/CBAS experience.
- A state issued driver's license, personal transportation, and auto insurance as required by law.

- Perform tasks that may involve bending, reaching, lifting, or assisting participants, depending on role responsibilities.
- Lift, carry, push, or pull supplies, equipment, or materials typically up to 25-50 pounds, with assistance or mechanical aids, depending on job duties and role responsibilities.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
