Reliant at Home is seeking a PRN Hospice RN Case Manager to cover Collin County and surrounding areas. The role includes comprehensive assessments, care planning, and patient-family education, with on-call responsibilities as needed. Qualified candidates hold a current RN license and CPR, with 2 years of RN experience in a clinical setting; hospice experience is preferred.
