Facility: Sylva Valley Health & Rehab (Skilled Nursing Facility)
Job Type: Full-Time (Part-Time/PRN considered)
Sylva Valley Health & Rehab is hiring an Occupational Therapist (OT) and/or Occupational Therapy Assistant (OTA) to provide high-quality therapy services in a skilled nursing setting. This role is ideal for clinicians who are passionate about helping residents improve independence, safety, and quality of life through meaningful, functional goals.

- Evaluate residents and develop individualized occupational therapy plan of care
- Provide skilled OT interventions focused on ADLs, transfers, safety, cognition, and adaptive equipment
- Document evaluations, progress notes, and outcomes to support skilled coverage and care needs
- Coordinate with PT/SLP, nursing, providers, and the interdisciplinary team
- Educate residents/families on safety strategies, home programs, and equipment
- Participate in care conferences/IDT and discharge planning as needed
- Provide OT treatment interventions under the supervision of an OT
- Document treatments and resident response/progress
- Communicate changes in condition to the supervising OT and care team
- Assist with resident/family education per plan of care
- Support discharge planning and continuity of care
- OT: Current Occupational Therapist license + graduate of an accredited OT program
- OTA: Current Occupational Therapy Assistant license + graduate of an accredited OTA program
- Strong documentation and communication skills
- SNF/rehab experience preferred (new grads considered
