NURSE PRACTIONER:
Under the direction of the Chief Medical Officer, the Nurse Practitioner is responsible for the professional, organizational, and administrative aspects of providing quality medical care to Clinics - Diagnostic Treatment Centers (DTCs) patients.
Nurse Practitioner (NP) will work in collaboration with primary care physicians to ensure proper oversight and coordination of patient care.
The role’s core responsibilities entail: gathering detailed histories, performing physical exam and conducting relevant diagnostic tests, diagnosing and treating common medical conditions within their scope of practice, managing patients with chronic illnesses and tailoring treatment plans accordingly, educating patients and their families about disease prevention, health promotion, and slow progression of chronic illnesses, referring to specialists when medically warranted, documenting accurate and complete
patient records in the Electronic Health Records (Athena), adherence to professional standard professional conduct, participating in quality improvement initiatives to enhance patient care and outcomes, and work
effectively within a healthcare team including healthcare professional, colleagues, patients and their families.
Geriatric, Primary Care, and Internal Medicine experience will be considered
*DEA License, and Medicare & Medicaid IDs
*Home care experience preferred or willing to do home care
*Orientation/training first week in the Bronx - required
