LPN Charge Nurse

Job Summary:

Responsibilities:

• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)

• Supervise and mentor LPNs and CNAs, ensuring that all staff members are providing seamless and effective care

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

• Maintain accurate and up-to-date patient records and reports

• Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care

Requirements:

• Current LPN licensure in the State of Connecticut

• 1+ year of experience as an LPN Charge Nurse or in a similar role

• Strong clinical and leadership skills with the ability to motivate and inspire staff

• Excellent communication and interpersonal skills with patients, families, and colleagues

• Ability to work effectively in a fast-paced environment with a focus on quality patient care
