Licensed Practical Nurse

*Responsibilities*

* Coordinates and collaborates with the Site Medical Director and members of the IDT in regard to Infection Control and Surveillance activities at the PACE Center.
* Aids in education to participants regarding all PACE services.

* Embraces the team concept and interdisciplinary care in the LIFE Program environment. Participates as an active member of the interdisciplinary team, determining the nursing and day center goals of the care plan; monitoring the participants progress toward goals; documenting in the electronic medical record in accordance with LIFE procedures; and working closely with the Clinical RN Case Manger to monitor key benchmarks for nursing and day center services
* Triages participants, by phone or face to face, eliciting appropriate information from participant or representative to make determination whether advice or appointment is appropriate, in accordance with LIFE standards, under the supervision of the clinic leadership.
* Assists with diagnostic and therapeutic procedures; records all vital signs as indicated by visit or follow-up standards; performs routine lab work; helps participants prepare for examinations according to departmental procedures and in a timely manner; maintains 100% accuracy in the performance of all routine tests, procedures and measurements according to standards (urine dipstick, blood glucose, ECG, nebulizer, etc.).
* Administers medications and applications, etc. as ordered by physician; maintains stock par level of all medications; monitors and maintains record of refrigerator temperatures; monitors medication storage area for expired and unused medication and supplies, etc.
* Responsible for the weekly distribution of participant medications, including medication reconciliation.
* Efficiently and accurately utilizes all equipment in the clinic, including but not limited to: EKG and suction machines, nebulizers, glucometer and oximetry; demonstrates knowledge of all emergency and safety equipment and practices good safety standards at all times.
* Educates participants regarding personal care and hygiene, medication and diagnostic tests as appropriate
* Respects participants right to privacy and confidentiality.
* Effectively assists with the orientation of others to the Albright LIFE program.
* Assesses, evaluates, and reports to physician the participants physical and emotional condition, reaction to medications and/or treatments, and/or critical laboratory result requiring PCP intervention.
* Only acts within the scope of the RNs authority to practice.
* Meets a standardized set of competencies for the specific position description established by the PACE organization before working independently.
* Independently performs skilled nursing procedures, assessments and techniques based on specific Participants needs while at the PACE Center.
* Evaluates and promotes a safe environment for the participant and complies with National Patient Safety.
* As assigned, covers on-call duties.
* All other duties as assigned.

*Qualifications*

* Must be a Licensed Practical Nurse currently licensed in the Commonwealth of Pennsylvania with one year experience as a licensed practical nurse with a frail or elderly population.
* Proven track record of strategic leadership, program expansion, and provider relations in a complex healthcare environment.
* Strong understanding of value-based care models, risk-based contracting, and regulatory compliance within Medicare/Medicaid frameworks.
* Minimum of one (1) year general clinical experience in a certified home health agency (CHHA), acute care, medical surgical, and/or critical care, Nursing Home experience preferred.
* Customer Service experience required.
* Minimum of 1 year of experience working with a frail or elderly population or, if the individual has less than 1 year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
* Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
* Exceptional customer service skills. Results-oriented; ability to handle multiple, time-sensitive projects and prioritize accordingly, while focusing on delivering required outcomes
* Strong communication and interpersonal skills written and oral; demonstrated ability to influence outcomes and work effectively with all organizational levels
