Therapist - Physical Therapist

Job Description

Job Summary:

Provides evaluate, develop, and implement individualized treatment plans for patients with physical functioning disorders, including wound care. Utilizes evidence-based practices to improve patient outcomes and enhance quality of life.

Responsibilities:

Screen and assess patients to determine their physical therapy needs and develop appropriate care plans.
Administer therapeutic procedures such as manual techniques, ambulation, exercises, modalities, supportive devices, and wound care.
Monitor patient progress, adjust treatments, and document outcomes in compliance with regulatory and clinical standards.
Educate patients, families, and caregivers on treatment plans, prevention strategies, and home exercises.
Collaborate with healthcare team members to coordinate patient care and ensure optimal recovery.
Complete discharge summaries and maintain accurate, timely documentation of services provided.

Qualifications:

Hold a valid MA State Physical Therapist license.
Demonstrate at least 1 year of clinical PT experience, preferably in LTC or rehab settings.
Possess experience with standardized assessments and evidence-based interventions.
Be able to supervise PT assistants, aides, and students as needed.
Exhibit strong communication skills and adherence to ethical standards governed by APTA and state regulations.
