Position Summary: This individual will play a leadership role in helping to shape a new Triage team in the Plymouth region. The intent of Triage is to reduce and end homelessness more rapidly than traditional shelter. Triage involves immediate assessment of all individuals seeking shelter, diverting them to alternative options or working toward their rapid exit to housing or treatment whenever possible. The coordinator will oversee day-to-day operations of the Triage team. This position will report to the Plymouth Stabilization Manager. This role will also help lead a cultural shift and change in operations toward innovative Triage programming in the Plymouth region. This position directly supervises the Re-Housing case manager, Outreach worker, and Winter Shelter Support Staff. This position is full-time, flexible hours, primarily afternoon/evening with some weekends and overnights required. Bachelor’s Degree and/or 3-5 years of increasing responsibility within a non-profit organization; 2+ years prior supervisory experience is required. Essential Responsibilities:

• Shelter Operations: Ensure smooth daily operations at each Winter shelter site. Ensure the safety and security of the guests, staff, and shelter site are met with adequate staffing and supervision. Monitor staffing needs and minimize use of overtime. Be prepared to de-escalate crisis situations and support staff in de-briefing difficult situations. Coordinate with the Plymouth Taskforce to End Homelessness for Winter Shelter needs related to volunteers, and site-specific issues.
• Triage Management: Provide oversight of the prevention, diversion, re-housing, and outreach work in the Plymouth/Wareham region. Work with the Stabilization Manager to review activities and the staff’s work

with guests and implementation of model changes. Help to Identify and implement changes in services that promote Triage goals and programming.

• Supervision: Hire, train, supervise, guide, and support the Triage staff. Provide individual and team supervision. Supervise and advise the Stabilization Manager in any issues or challenges related to staff. Address personnel issues in collaboration with Human Resources and other staff.
• Client recordkeeping: Establish and ensure that monitoring of client (guest) recordkeeping is occurring within the program. Maintain the privacy of all guest information in accordance with agency policies, procedures, and practices, as required by law, and in accordance with general principles of professionalism as a social service provider.
• Data collection and reporting: Track contract adherence and goals. Prepare monthly and other progress reports to ensure programs are meeting goals and objectives. Ensure that files are up to date and in compliance with all State and federal contracts. Interface with funders, identifying and resolving any concerns. Conduct file reviews, obtain monthly reports from managers. Work closely with Data Team to conduct periodic checks of data entry in the FBMS database system.
• Community relations: Maintain knowledge about area vendors, partners, providers, and property. Maintain relationships with, and cooperate with, various community providers to access services for guests.
