CASE MANAGER

The Rite of Passage Team is hiring for Case Manager at Yosemite Treatment Center in San Andreas, California. Nestled in the Sierra foothills of California, the Yosemite Treatment Center located on the Sierra Ridge Campus is a licensed short‑term residential therapeutic program that offers individual, group, and family therapy, as well as evidence‑based skill and rehabilitation groups. The center embraces a strengths‑based approach within a least restrictive program model and a normalized high‑school environment. Through Rite of Passage’s Integrated Care Model, the center provides a trauma‑responsive program individualized for each youth. Pay Starting at $24.00 per hour. Perks & Benefits Medical insurance beginning the first of the month after 60 days of full‑time employment. Full benefit package after 90 days of full‑time employment. 403(b) match of up to 6% after one year of employment. Paid time off (PTO) usable as soon as it accrues. Responsibilities Administrate, develop, and facilitate the completion of case plans and treatment programs. Develop and oversee the completion of the MAP Plan. Provide monthly progress notes and weekly student updates. Facilitate weekly group meetings. Conduct assigned responsibilities with respect, consideration, encouragement, positive guidance, and utilization of resources toward the youth’s overall success. Qualifications Empathetic, patient, consistent, compassionate, with an honest work ethic and respectful demeanor. Desire to work with troubled teens. Bachelor’s degree in a related field. Prior case management experience, including at least 3 years with youth and/or juvenile placement agencies. At least 21 years of age. Ability to pass a criminal background check, drug screen, physical exam, TB test, and a search of the child abuse central registry. Schedule 5 days per week: Sunday to Thursday or Tuesday to Saturday. 8‑hour shift with a 1‑hour break.
