This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator.Core responsibilities include:Conducting comprehensive occupational therapy evaluations and re-evaluations in accordance with physician orders and California standards of practiceDeveloping, implementing, and modifying individualized plans of care with measurable, function-based goalsDelivering skilled therapeutic interventions addressing ADLs, IADLs, neuromuscular re-education, upper extremity rehabilitation, cognitive retraining, sensory integration, visual-perceptual skills, and adaptive strategiesAssessing and recommending adaptive equipment, assistive devices, splints, orthotics, and environmental modifications to enhance independence and safetyFabricating and training patients in the use of splints or adaptive devices as clinically indicated and within scope of practiceMonitoring patient progress and modifying treatment plans based on clinical response and objective outcomesReviewing patient medical history, conducting patient interviews, and observing functional task performanceEvaluating patient home or workplace environments and recommending modifications based on health and functional needsLeading discharge planning and transition-of-care coordination to promote successful reintegration into home and community environmentsCommunicating evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, patients, and familiesEducating patients, families, employers, and caregivers on how to accommodate and support the patient's functional needsSupervising and directing Occupational Therapist Assistants in accordance with California state law and professional regulationsProviding appropriate oversight to rehabilitation aides, students, and volunteers as assignedCompleting timely, accurate, and compliant documentation consistent with federal, state, payer, and organizational requirementsParticipating in care conferences, quality assurance initiatives, and collaborative planning as requestedMaintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standardsTreating all residents with dignity, respect, and compassionate professionalismPhysical requirements include the ability to assist with patient transfers, upper extremity handling and positioning techniques, mobility assistance, repositioning and supporting patients with or without assistive devices, prolonged standing, walking, bending, reaching, kneeling, and stair navigation. The ability to assist in resident evacuation during emergency situations is also required.What You BringCurrent, active, and unrestricted Occupational Therapist license in the State of California — required (pending licensure considered)Graduate of an accredited Occupational Therapy program (Bachelor's, Master's, or Doctorate) — requiredSuccessful completion of the National Board for Certification in Occupational Therapy (NBCOT) — requiredCurrent CPR/BLS certification (or willingness to obtain per facility policy)Ongoing compliance with continuing education requirements established by the California Occupational Therapy BoardExperience in a skilled nursing, post-acute, hospital, or related medical setting — strongly preferredExcellent verbal and written communication skills with the ability to exercise sound clinical judgmentComposure and professionalism in dynamic, fast-paced clinical environmentsStrong organizational skills and the capacity to make independent clinical decisionsA team-first mindset and a genuine passion for skilled nursing and post-acute careEqual Opportunity and AccommodationGreenridge Post-Acute and Senior Living is proud to be an Equal Opportunity Employer.
