Field Case Manager: Housing & Safety Coordinator

Foothill Family

Foothill Family is seeking a Safeguard Case Manager to provide short-term, client-centered care coordination for clients with complex medical, psychosocial, housing-related needs. The role emphasizes risk mitigation, housing stabilization, and respectful, rights-based care. You will conduct comprehensive assessments, develop individualized plans, and collaborate with medical, behavioral health, and housing partners across Los Angeles County.
