You will embark on various responsibilities that directly impact patient care:

  • Conduct comprehensive psychosocial assessments to evaluate patients’ emotional, social, and financial needs, ensuring they receive the appropriate support.

  • Provide individual and group counseling sessions to patients and their families, addressing issues such as adjustment to illness, grief, loss, and coping strategies.

  • Facilitate collaboration with interdisciplinary medical teams to establish comprehensive care plans that encompass the psychosocial dimension of patient care.

  • Manage crisis intervention and support services, ensuring timely assistance during critical moments.

  • Coordinate post-discharge services by liaising with community organizations, thereby promoting seamless transitions and long-term care continuity.

  • Document patient interactions and care plans accurately and in compliance with legal and organizational policies.

  • Participate in hospital committees to contribute social work expertise and advocate for innovations in patient care practices.

Patient Population / Client Focus\n

This position primarily focuses on patients with life-limiting illnesses within a hospice framework, addressing their unique psychosocial needs. As an LCSW, you will provide essential emotional support to patients and their families, helping them navigate end-of-life issues, grief, and the multifaceted challenges that arise during this phase of care.

Qualifications\n

To be successful in this role, candidates should possess:

  • A Master’s degree in Social Work from an accredited program.

  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
