Licensed Practical/Vocational NurseThe Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.Primary Responsibilities:Provides direct patient care as defined in the state Nurse Practice Act and agency's policiesImplements plan of care initiated by the Registered NurseProvides accurate and timely documentation consistent with the plan of careAssesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of careParticipates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the Registered Nurse and/or Clinical SupervisorFollows all infection control standard precautions and safety guidelines/standards as per agency policyUses equipment and supplies effectively and efficientlyParticipates in personal and professional growth and developmentAdheres to the Agency's Standard Operating Procedures as it relates to the submission of the documentationParticipates in on-call duties within the on-call rotation schedule, to include weekends as assignedDemonstrates knowledge and observance of Patient Rights and Notice of Privacy PracticesEnsures that arrangements for equipment and other necessary items and services are availableMaintains a daily patient case load and point of care documentation levels, as per Agency standardsRequires the availability of accepting patients during business hours (8 a.m. - 6 p.m.)Requires updating the time card daily, as per UHG PolicyRequires contacting patients the night before the scheduled visit to provide an arrival time within a two (2) hour windowRequires obtaining pre-approval from direct supervisor for overtimeRequires the ability to work flexible schedule to meet patients' needsDemonstrates personal responsibility with regard to attendance and punctualityMaintains privacy and confidentiality with regard to all patient, staff, and agency informationDemonstrates flexibility, enthusiasm and willingness to cooperate while working with others or in place of others, as necessaryExpresses verbal and written communication in a clear, positive and cooperative mannerPromotes the agency's image by adhering to the agency's dress codePerforms all other related duties as assignedCommunication: Prepares clinical notes and updates the primary physician when necessaryCommunicates with the physician regarding the patients' needs and reports any changes in the patients' condition; obtains/receives physician's orders as requiredCoordinates services and schedules with the Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC/Scheduler) to include recommendations for additional home health care services for patients by the end of the work dayYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Graduate of an accredited practical nurse or vocational nursing programHigh school diploma or equivalentCurrent unrestricted license in the state of Texas as a Licensed Vocational Nurse (for Texas employees)Maintain current CPR certification2+ years of clinical experience preferable in a community home health or medical/surgical settingDemonstrates thorough knowledge of current nursing practice, professional standards of care and state and federal regulations regarding home careValid Texas (for Texas employees) Driver's License and maintain personal auto insurance coverage in accordance with organization requirementsThis position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of diseasePhysical and Mental Requirements:Ability to lift up to fifty (50) pounds (must be able to lift and transfer patients from one location to another, if necessary)Ability to push/pull heavy objects using up to fifty (50) pounds of forceAbility to sit for extended periods of timeAbility to stand for extended period of time.Ability to use fine motor skills to operate office equipment and/or machinery.Ability to properly drive and operate a personal/company vehicle.Ability to receive and comprehend instructions verbally and/or in writing.Ability to use logical reasoning for simple and complex situationsWork Environment (Patient's Home):The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The hourly pay for this role will range from $20.00 - $36.00 per hour based on full-time employment.
