Registered Nurse - RN Case Manager

Join a dynamic team as a Registered Nurse Case Manager, providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses caused by exposure to toxic substances. Responsibilities include: conducting weekly in-home nursing visits, performing physical assessments, ensuring home safety, managing medication plans, and overseeing caregiving staff. Collaborate with regional staff to improve services, coordinate care, and participate in clinical and performance improvement activities. Support staffing, training, documentation review, and quality assurance efforts. Engage in community outreach and marketing activities as needed.
