Home Health Speech Language Pathologist-PRN

Overview As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence from the comfort of their homes. You will deliver personalized care focusing on rehabilitation and functional improvement, empowering individuals to overcome physical limitations and enjoy a better quality of life. Key Responsibilities Evaluate, direct and provide speech/language pathology services to patients in the home or facility. Participate in the development and periodic review of the Plan of Treatment and Plan of Care. Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects of such disorders and conditions. Administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening. Observe, record and report changes in patients’ condition and response to treatment to supervisor and/or physician. Provide instruction and training to patients in use of alternative communication systems when appropriate. Provide counsel and instruction to patients, families and healthcare staff. Maintain and submit documentation as required by the Company and/or facility, including written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports in accordance with Company policy. Participate in care coordination activities and discharge planning. Maintain high standards of professional conduct regarding confidential information; share information only when access rights are established and in the best interests of the patient. Required Qualifications Minimum six months experience as a speech therapist / speech language pathologist. Current and unrestricted license. Current CPR certification. Valid driver's license, auto insurance and reliable transportation. Preferred Qualifications Home Health experience. Compensation Pay Per Visit/Unit Rate: $54.00 - $76.00 Scheduled Weekly Hours: 1 Pay Range: $59,300 - $80,900 per year Additional Information TB Statement: This role is patient facing and is part of Humana’s Tuberculosis (TB) screening program. Candidates will be screened for TB. Driving Statement: This role is part of Humana’s driver safety program and requires a valid state driver’s license. Candidates must maintain personal vehicle liability insurance in accordance with state requirements.
