Job Summary:

The PATH Care Manager provides resident-centered case management, support, and advocacy to individuals living at Providence at the Heights (PATH). This role partners with the residents, to develop and maintain individualized goal plans that promote long-term housing stability, wellness, and positive health outcomes within a Permanent Supportive Housing model. The PATH Care Manager completes needs-based assessments, coordinates services, connects residents to income resources and public benefits, and supports crisis response, conflict resolution, and community-based service navigation. This position is based at PATH, 15650 E. Alameda Parkway, Aurora, Colorado, and reports to the Program Manager/Associate Director of Housing.

Duties/Responsibilities:

• Establish and maintain regular contact with assigned residents based on individual needs, goals, and housing stability factors.
• Partner with residents to develop, implement, and update individualized goal plans that reflect resident-identified priorities and objectives.
• Conduct needs assessments and provides case management, service coordination, advocacy, and problem-solving support.
• Assist residents with access to public benefits, income resources, community services, healthcare, and behavioral health supports.
• Provide transportation or transportation coordination for residents when required and appropriate.
• Serve as an effective mediator and advocate while fostering respectful, professional relationships with residents, staff, and community partners.
• Attend weekly case management meetings and all required training, consultations, and program meetings.
• Document resident interactions, service plans, referrals, and outcomes accurately and in a timely manner.
• Deliver high-quality, trauma-informed services that build trust, collaboration, and community within an interdisciplinary Permanent Supportive Housing setting.
• Increase residents’ access to and engagement in mental health and other supportive services through close collaboration with property management, internal teams, and external providers.
• Assess crisis situations, use de-escalation techniques when needed, contact appropriate authorities when necessary, and document incidents in accordance with program requirements.
• Remain current on best practices in Permanent Supportive Housing, Housing First, harm reduction, and trauma-informed care.
• Perform other duties as assigned by the Program Manager 

Required Skills/Abilities:

• Excellent oral and written communication skills, with the ability to communicate effectively with residents, staff, families, partners, and service providers.
• Strong organizational skills and attention to detail.
• Ability to manage multiple priorities in a fast-paced and at times high-stress environment.
• Excellent time management skills and a demonstrated ability to meet deadlines.
• Strong critical thinking, problem-solving, and conflict-resolution skills.
• Ability to work effectively both independently and as part of an interdisciplinary team.
• Ability to maintain professional boundaries and practice trauma-informed, culturally responsive service delivery.
• Proficiency in Microsoft Office applications, including Word, Excel, and Outlook, and the ability to learn required data and documentation systems.

Education and Experience:

• Bachelor’s degree in social work, psychology, human services, counseling, or a related field required.
• Experience providing case management or direct services to individuals or families experiencing homelessness, mental health conditions, and/or substance use disorders.
• Experience in Permanent Supportive Housing, Housing First, harm reduction, or trauma-informed care strongly preferred.
• Experience serving adults transitioning from incarceration and/or adults experiencing homelessness strongly preferred.
• Experience in crisis intervention, behavioral health support, and community resource navigation preferred.
• Demonstrated ability to serve people from diverse backgrounds with respect, cultural humility, and professionalism.
• Ability to collaborate effectively with internal teams, property management staff, and external providers.

Physical Requirements:

• While performing the duties of this job, the employee is occasionally required to stand, walk, sit, and use hands and fingers to handle or feel objects, tools, or controls. 
• Reach with hands and arms, climb stairs, talk, or hear. 
• The employee must occasionally lift or move office products and supplies up to 20 pounds.

Compensation & Merit Increases:

1. Rate: $23.08 - $24.04 / hour
2. Vacation Leave & Sick Leave, Medical, Dental & Vision Insurance, 401k Plan.

Position Type/Expected Hours of Work

• This is a full-time position requiring a minimum of 40 hours per week. 
• This is a results-driven role based primarily on site, with limited flexibility for scheduled remote work as approved. Some evening and Saturday hours may be required to meet program needs.
