Responsibilities
- Conduct initial assessments and ongoing evaluations of residents’ functional status, ADLs, mobility, and cognitive needs.
- Develop and implement individualized OT treatment plans aimed at improving independence in daily activities and mobility.
- Reassess progress regularly, adjust goals, and coordinate with the interdisciplinary team for care planning and discharge planning.
- Provide education to residents and caregivers on adaptive equipment, activity modification, safety, and home programming to support continued independence.
- Document patient status, progress, and outcomes in a timely, compliant manner and ensure alignment with facility policies and regulatory standards.

Requirements
- Current Occupational Therapist license (or eligible for licensure) to practice.
- Ability to participate in a 13-week travel assignment in the designated location.
- Strong communication skills and ability to document and collaborate within a multidisciplinary team.
- Experience in acute care or skilled nursing settings is preferred.
- Commitment to patient safety, evidence-based practice, and adherence to regulatory and facility policies.
