This role is about providing in-home care and support for our senior patients and serving as their clinical liaison with our primary care providers. The Medical Technician Partner will work collaboratively with our primary care providers, Gather Guide Partners, and clinic administrators, as part of the care-team model. The MTP will play a significant role in our ability to provide the best care and support possible with a deep sense of commitment and compassion and ultimately achieve our vision of creating equity in healthcare.

The day-to-day duties will vary, depending on the needs of our patients. Some of the day-to-day duties will include:

• Providing in-home care and support for our senior patients and serving as their clinical liaison with our primary care providers
• Performing approximately six to eight home visits per day and addressing acute medical decompensation or provide routine clinical care including phlebotomy and point of care testing (EKGs, UAs, INRs, respiratory swabs, etc.)
• Check in and organize an inpatient or ED admission avoidance plan
• Conduct social visits with their patients in their homes
• Make outbound calls to patients to check in on health status
• Leverage and access medical control through the Clinical Medical Director as needed to deliver care
• Utilize Electronic Health Record (Athena) to review patient appointments, charts, and document notes
• Additional duties as assigned

We offer flexible schedules for Partners to fit your needs - 4 or 5 days per week for full time positions.
