Growing together.Reliant has an immediate opening for a friendly, patient focused and detailed oriented Associate Patient Care Coordinator to join our team. The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.Schedule: Monday - Friday, 8:30am - 5pm. This is a hybrid role; after initial 12 weeks of training are completed onsite, will be working 2 days onsite and 3 days remote. Will be required to work onsite 5 days per week for 12-week training.Location: 5 NEPONSET STREET WORCESTER, MADepartment: Behavioral HealthPrimary Responsibilities:Verifies and updates demographics and other patient information including necessary consent and other documentsSchedules patients accurately, according to department protocols and provider preferenceCompletes patient check-out, coordinates scheduling, and other services as directed. Managers and updates the Department Appointment ReportProvides education, estimate preparation, and other pre-appointment instruction as neededResolves patients' questions regarding clinic schedules and billing concerns; handles requests for prescription refills, reschedules appointments, etc.Accurately and appropriately documents information in the electronic medical record systemReceives and processes incoming and outgoing insurance referral requests based on department protocol.Collaborate with staff, patients, and providers as needed to complete requests. Complete follow-up and documentation as neededProcesses personal in-basket and department pool encounters in Epic in a timely and appropriate mannerProcesses department and provider mail and faxes in a timely and appropriate mannerAble to identify when patient-related issues need urgent attention and escalate to nursing or provider and/or leadership as neededComplies with health and safety requirements and with regulatory agencies such as DPH, etc.Complies with established departmental policies, procedures, and objectivesParticipates in learning opportunities provided through the learning management systemParticipates in departmental meetings and huddles as directed by department leadershipRegular, reliable, and predictable attendance is requiredAbility to be cross trained to be able to work in the team room and check outPerforms other similar and related duties as required or directedWhat are the reasons to consider working for UnitedHealth Group? The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment.
