Our client is looking for an experienced pharmacist to join their home infusion team in Rochester. If you have a background in acute care or home infusion and are looking for a clinically focused opportunity with a consistent schedule, this could be a great fit.
What You’ll Do
Review infusion orders and patient therapies
Participate in clinical rounds
Perform sterile compounding and TPN review
Maintain patient records and care plans
Ensure USP and cleanroom compliance
Train and support staff
Drive operational improvements
What We’re Looking For
Current licensure as a Registered Pharmacist in the State of New York.
Minimum of two years of recent experience in an acute care setting or one year of recent home infusion therapy experience preferred.
Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
Ability to evaluate and monitor drug therapy relative to the total needs and clinical conditions of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
