This position provides expertise and leadership to ensure effective resource management for patient care delivery, focusing on complex pediatric cases with up to ten or more patients daily. The RN Care Manager plays a vital role in enhancing continuity of care, managing patient flow, discharge planning, resource utilization, and maintaining compliance with regulatory standards. Key responsibilities include managing inpatient and observation patients through assessment, planning, intervention, and evaluation; collaborating with the healthcare team to develop and implement discharge strategies; monitoring length of stay to optimize care efficiency; ensuring proper documentation and compliance with care guidelines; facilitating communication with payers and providers; and participating in interdisciplinary rounds. The ideal candidate should have a minimum of 5 years of clinical practice, RN licensure in Texas, preferably with case management experience, and proficiency with clinical guidelines such as MCG/InterQual.
