Patient Engagement & Enrollment Specialist

Upward Health is seeking a Community Outreach Specialist to establish the company’s presence in the community and educate potential patients about our in-home, whole-person care services. This role involves phone outreach, in-person visits, and various community-based strategies to engage individuals and enroll them into programs. The COS will manage a personal caseload, report to the Outreach Manager, and work with interdisciplinary teams to ensure successful outreach and patient onboarding. #J-18808-Ljbffr
