ABOUT THE JOB

As a Case Manager on the Outreach Team, you will assist clients in breaking the cycle of homelessness by employing a “whatever it takes” approach. Your main responsibilities include assessing needs, developing Individualized Service Plans, providing referrals, and maintaining accurate case files.

POSITION RESPONSIBILITIES

Conduct intake and person‑centered needs assessments for all participants and collaborate to develop Individualized Service Plans (ISPs) that address housing goals, income, and other personal goals such as medical, mental health, substance use, financial resources, vocational, and social support needs. Provide referrals, linkages, information, and support to resources that help participants achieve their ISP goals. Complete program‑specific assessments (e.g., VI‑SPDAT). Evaluate strengths and challenges to address short‑term and long‑term goals, and conduct a 90‑day review with the participant after initial intake. Maintain documentation standards as required by the Program and PATH policies, completing case file notes in a timely manner and ensuring a current, complete participant file with all required documents. Maintain confidentiality of participant files per HIPAA and applicable guidelines.

Communication: convey information clearly and effectively, both verbally and in writing, to individuals and groups. Critical thinking and problem solving: analyze information and situations, identify key issues, consider alternatives, and make logical decisions that achieve desired outcomes. Ethics and boundaries: maintain professional standards, demonstrate honesty, integrity, and a commitment to ethical conduct while respecting professional boundaries.

PREFERRED QUALIFICATIONS

Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations.

MINIMUM QUALIFICATIONS

All levels of education and experience are welcome.

MANDATORY REQUIREMENTS

Employment eligibility verification. CPR/First Aid training or willingness to obtain. Ability to complete the following as a condition of hire: Tuberculosis test Background screening Drug test

DRIVING

Have reliable transportation. Maintain a valid driver’s license. Provide proof of insurance and ownership for personal vehicles used during work duties. Qualify for PATH’s insurance coverage.

EMPLOYMENT ELIGIBILITY

CALIFORNIA‑BASED WORK EXPECTATIONS

Work must be performed within California. PATH does not offer fully remote positions; some roles may involve hybrid schedules but not work from home.
