Job Description
Job Title: Dialysis Nurse

Experience: Minimum six months in a chronic dialysis facility preferred
License: Current state Registered Nurse Licensure within the state of Utah


Description:
Deliver care to patients using the nursing process of assessment, planning, intervention, implementation, and evaluation.
Round with physicians, implement, and follow up on orders.
Collaborate with other professional disciplines to ensure effective patient care.
Assist before and during dialysis procedures, following the physician's orders.
Oversee dialysis technicians as they perform daily duties.
Supervise dialysis of chronic renal failure patients.
Help establish a plan of care for each patient and provide consistent, effective dialysis according to the plan.
Provide follow-up of medical orders for each patient.
Provide knowledge and assistance in adjusting each dialysis to each patient's particular physiologic imbalance.
Assist in evaluating the social integrity of each patient and recommend interventions as needed.
Set goals (immediate and future) for the patient, planning these goals with the patient, physicians, social workers, dietitians, staff nurses, and technicians.
Periodically review these goals with the same team.
Ensure safe dialysis by checking and testing supervision of dialysis equipment and supplies before initiation.
Monitor patients and equipment throughout dialysis.
Ensure cleaning and disinfection of equipment and stocking of supplies.
Maintain all equipment in a safe and efficient working order.
Instruct patients in self-care to prevent complications between dialysis treatments.
Record and communicate information about patient records and plan of care to other healthcare personnel.
Transfer this information if the patient is admitted to the hospital or transferred to another outpatient center.
Supervise recognition of emergencies, remain knowledgeable, efficient, and provide instruction in emergency care.
Ensure emergency equipment is available and ensure immediate care to correct emergency conditions.
Involve the patient in their care by providing education on dialysis and related topics.
Teach about dietary and fluid management, medications, chronic renal failure, and the role of dialysis.
Facilitate coordination with dietitians.
Assist in plans for unit changes.
Order medications and supplies, ensuring that the unit is well stocked with all necessary equipment.
Attend staff meetings at least 80% of the time or review minutes with the dialysis manager.
Complete all required education.
Ensure that appearance and personal conduct are professional at all times.
Maintain an excellent attendance record.
Wear appropriate clothing for job functions and an ID badge.
Work to maintain a good rapport and a cooperative working relationship with physicians and staff.
Represent the organization positively and professionally in the community.
Maintain patient confidentiality at all times.
Comply with all organizational policies regarding ethical business practices.
Follow all human resource policies.
