Responsibilities
- Conduct OT evaluations and develop therapy plans addressing activities of daily living, mobility, cognition, and functional independence for patients in a skilled nursing setting.
- Implement individualized treatments and modalities to improve performance with daily tasks and safety.
- Document initial assessments, progress notes, and discharge plans in the patient record, ensuring regulatory compliance.
- Monitor patient progress, modify treatment plans as needed, and provide clear communication to the care team.
- Collaborate with physicians, nursing staff, physical therapy, social work, and other specialists to coordinate comprehensive care and smooth discharge planning.
- Educate patients and families on home exercise programs, safety strategies, and coping techniques to support continued recovery after discharge.

Requirements
- Active occupational therapy license (as applicable per Job Notes)
