Field-Based HIV Outreach Case Manager

Boston Health Care for the Homeless Program is seeking an Outreach Case Manager to join the HIV Primary Care Team. You will provide community-based outreach, linkage, and retention services to engage patients in HIV care and support ongoing treatment in BHCHP settings. The role works with Outreach Nurses and a Social Worker to reach vulnerable individuals facing homelessness, housing instability, and barriers to care, including insurance and benefits, with a patient-centered, strengths-based #J-18808-Ljbffr bhchpjobs
