Roze Room is a leading provider of Hospice and Palliative Care, celebrating over 25 years of service to Southern California communitiesPosition OverviewThe Clinical Care Coordinator (LVN) serves as a liaison between patients, families, referral sources, and the hospice/palliative care team. This field-based role provides education, conducts nursing assessments, coordinates admissions, and helps ensure patients receive timely, compassionate care.ResponsibilitiesConduct nursing assessments and admissionsReview consents along with other documents and assists with election of hospice benefit.Educate patients and families about hospice and palliative careCoordinate care transitions from hospitals and facilitiesCollaborate with interdisciplinary teamsComplete documentation using EMR systemsQualificationsCurrent California LVN License1+ Year Nursing Experience, hospice experience highly preferredStrong Communication SkillsReliable Transportation & Valid Driver's LicenseBilingual Spanish PreferredJoin a team committed to providing comfort, dignity, and exceptional care when it matters most.
