Job Description Reporting directly to the Practice Manager and Senior Director of Nursing, the Team Nurse RN position is responsible for performing the duties of an RN in an outpatient primary care or family medicine practice and serves as a clinical resource for the medical practice. The Team Nurse RN provides high quality, direct patient care in a team‑based model to our Beth Israel Deaconess HealthCare (BIDHC) patients. The Team Nurse RN is responsible for enhancing the safety, effectiveness and efficiency of patient care through the support and delegation of clinical and administrative functions as appropriate to the Medical Assistant (MA) staff. Responsibilities include educating and training the MA team and patients on general and focused health education for preventive, chronic and episodic health‑care issues, collaborating with primary care providers to coordinate patient‑centered care, supporting transitions between levels of care, and counseling, educating and supporting patients in improving health status. Primary Responsibilities Coordinate and facilitate quality clinical management of a broad‑based outpatient population across the continuum of care. Collaborate with providers to identify needs for preventive care, evidence‑based chronic disease management and care coordination. Provide general and specific health‑care education. Work directly with patients to identify and monitor self‑management goals. (essential) Collaborate with community‑based providers. Follow up on ED visits and inpatient discharges. Assist providers with exams, treatments and procedures as needed. (essential) Provide skilled telephonic triage and assessment, education, collaboration and coordination of health‑care and community resources. (essential) Perform direct patient care, including Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) of BIDHC patients. (essential) Responsible for INR management and teaching for patients on Coumadin using a standardized protocol. (essential) Administer vaccines and medications by parenteral, oral, rectal or nasal route by orders of the prescribing provider and by the Medication Administration policy guidelines established by BIDHC. Provide patient education as appropriate for the administered medication. (essential) Perform a wide range of nursing activities as needed under the direction of the provider such as wound care, point‑of‑care testing, etc.; accurately document all aspects of patient care in the Electronic Health Record in a timely manner; schedule tests and referrals for specialty care on order of provider. (essential) Monitor and ensure follow‑up of all abnormal test and imaging results under the direction of the provider. (essential) Oversee the inventory and ordering management of clinical supplies and equipment as necessary; ensure safe, accurate and clinically appropriate collection, storage and processing of all biological specimens. (essential) Required Qualifications Graduate from an accredited Nursing Program; Bachelor’s degree preferred. Licensed Registered Nurse in Massachusetts; Basic Life Support certification required. 1–3 years related work experience required; new graduate RNs will be considered. CPR Certification. Strong interpersonal skills; strong patient communication and education skills. Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications; capable of producing complex documents, performing analysis and maintaining databases. Daily protective equipment required (respirator, earplugs, mask, gloves, eyewear, etc.); potential exposure to infectious diseases, airborne and bloodborne pathogens. Health Care Worker status: regular, day‑to‑day contact, face‑to‑face and hands‑on within three feet for at least five minutes; examples include physicians, clinical nurses, phlebotomists, medical assistants, PFT technicians, x‑ray technicians. Sensory Requirements Must be able to perform close work (paperwork, visual examination); must possess mandatory color vision and perception; must tolerate visual monotony; must have visual clarity > 20 feet and Physical Requirements Medium work: Exert up to 50 pounds of force occasionally and/or up to 20 pounds frequently; physically demands standing and/or moving throughout the shift. Constant sitting, walking, endurance working up to 3–4 hours without a break, frequent standing, bending neck or waist, power grasping, fine manipulation, pushing/pulling, keyboard use, reaching above and below shoulder height; may occasionally push a wheelchair or wheeled bed carrying up to 250 pounds with assistance; rarely must pull or reposition patients weighing up to 100 or 250 pounds. Pay Range $32.24 – $55.32 per hour. Compensation may exceed the base hourly rate based on factors such as seniority, education, training, experience, certifications, location, responsibilities and other lawful factors.
