Nurse Practitioner/Physician Assistant

We are seeking a Nurse Practitioner or Physician Assistant to join an established adult primary care practice within a collaborative team including an experienced physician and advanced practitioners. Key responsibilities providing comprehensive primary care for adults, conducting wellness visits, screening, and routine exams, diagnosing and treating acute and chronic conditions such as hypertension, diabetes, and COPD, and developing personalized treatment plans. The role involves ordering and interpreting diagnostic tests, educating patients on medications and lifestyle changes, coordinating care with the team, utilizing telemedicine and patient portals, and maintaining accurate electronic health records. Qualifications current New York State licensure, graduation from an accredited NP or PA program, DEA registration or eligibility, and strong clinical assessment skills. Preferred candidates have experience in Internal Medicine or Primary Care and managing chronic diseases, but new graduates with a passion for adult primary care are encouraged to apply. The position offers a full-time outpatient schedule, access to on-site lab and specialty services, integrated electronic records, and a patient-centered approach with supportive, physician-led organization.
