OhioRISE Navigator - Inpatient & Community Transitions Location: Delaware, OH, US Employment Type: Full-time Regular Professional Salary Range: $49,895.00 – $60,114.00 Annually Requisition ID: 2490 Opening Date: 26 days ago About the Role The OhioRISE Navigator is a mission‑critical role that strengthens system coordination, improves access to care, and enhances outcomes for youth and families by ensuring timely, efficient, and well‑supported transitions from inpatient to community‑based services. Responsibilities Coordinate discharge planning and transitions for patients from inpatient psychiatric and medical units to community services. Collaborate with hospital staff, community providers, and families to develop individualized care plans. Monitor and support continuity of care during the transition period. Provide education and training to providers, staff, and families regarding transition processes. Maintain accurate and timely documentation of transition activities. Advocate for patients and families to ensure access to resources and services. Minimum Qualifications Bachelor’s degree in social work, healthcare administration, nursing, or related field and/or equivalent experience. Minimum of 2–3 years of experience in behavioral health, healthcare systems, or care coordination. Knowledge of hospital systems, emergency departments, and discharge planning processes. Strong organizational, communication, and problem‑solving skills. Preferred Qualifications Experience with OhioRISE, Medicaid systems, or managed care organizations. Experience in care transitions, utilization management, or system navigation. Knowledge of Aetna MAF processes, authorization workflows, or referral management. Experience providing education or training to providers or staff.
