Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide care in a Skilled Nursing Facility / Long Term Care setting. The LPN will work with a mix of long-term residents and those in recovery, ensuring a high standard of care across a 30-bed unit with private rooms.

Responsibilities

• Administer medications as prescribed
• Provide wound care and ostomy care
• Manage foley catheters and PEG tube feedings
• Maintain patient records and EMR documentation using Point Click Care
• Collaborate with nursing staff and interdisciplinary teams

Required Experience / Certifications / Licensure

• Current LPN license
• CPR certification obtained prior to the first day of work
• Experience in a skilled nursing facility is preferred
