About the Role

This is a Full-Time position in Evergreen, Colorado with a Monday-Friday workweek from 8:30AM – 5:00PM. As a Physical Therapist, you’ll be a vital member of our Home Health team, providing compassionate, high-quality care throughout our mountain service area.

In this role, you will:

• Provide skilled, home-based physical therapy to patients
• Develop and adjust individualized care plans with measurable goals
• Educate patients and caregivers on recovery, safety, and long-term wellness
• Collaborate closely with RNs, Social Workers, Home Health Aides, and Leadership to deliver holistic, patient-centered care
• Primary service area would be the Foothills area of Jefferson County.
• Maintain timely, accurate clinical documentation

Work Environment

• Daily travel throughout our mountain service area (this is a field-based role)

Compensation & Benefits

• $46.00–$50.00/hour, based on experience
• $5000 Sign-on bonus

What You Bring

Required:

• PT or DPT degree
• Active Colorado Physical Therapy license
• Minimum 2 years of experience
• Strong clinical, communication, and time management skills
• Ability to work independently while contributing to a team
• Valid driver’s license, reliable vehicle, and insurance
• Legal authorization to work in the U.S.
• COVID-19 vaccination (or approved exemption)

Preferred:

• Home Health experience
