About the Position

TravelNurseSource is working with Malone Healthcare to find a qualified Case Manager RN in Eureka, California, 95534!

• Provide case management services within an acute care hospital setting
• Coordinate patient care planning, discharge planning, and utilization review
• Float between like units based on facility need
• Document and chart patient information using Epic EMR
• Work every other weekend as required by the facility
• Commit to working 3 of 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day

• Active RN license required; Compact license must be in hand at time of submission
• Minimum of 2 years of nursing experience required; at least 1 year of case management or acute care experience required
• Acute care experience is required
• Must submit all requested time off (RTO) at the time of initial submittal - post-offer RTO requests may not be granted
• 2 professional references covering at least 1 year of experience within the last 3 years required prior to start
• Local candidates (within 50 miles of the facility) are accepted

• Prior experience with Epic EMR charting system
• Experience in hospital-based case management or care coordination
• Flexibility to float between sister facilities as needed

• Located in Eureka, CA - easy driving distance to 7 state and national parks, offering access to the rugged California coast, the famous Redwood Highway, and unique Victorian architecture throughout the city
• Housing assistance resources may be available through the facility for travelers who need support securing accommodations
