Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans based on the patient's needs, goals, and physician directives. This role includes overseeing the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective community reintegration.

The therapist also serves as an educator for patients, families, students, coworkers, and themselves, performing other professional duties associated with patient care and the development of discipline-specific therapy services. The role emphasizes compliance with the organization's Mission Statement, Core Values, and Code of Conduct, maintaining confidentiality regarding patient and company information, and upholding professional codes of conduct.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans focused on patient progress towards discharge.
• Document medical records clearly, accurately, and in compliance with applicable guidelines and clinic policies.
• Participate in student programs, providing supervision and development of learning objectives.
• Create an individualized professional development plan and pursue ongoing professional growth through continuing education.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and contribute to maximizing outcomes.

Required Experience / Certifications / Licensure

• Bachelor's or Doctorate degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR Certification.
• Active and applicable state license for Physical Therapy.
• Previous successful assignment completion with the agency is highly desired.

Schedule

This is a 5x8.0 schedule, starting on 8/31.

Assignment Duration

This is a 13-week contract position.
