What you will do in this role:

• Assist and coordinate intake and pre-admission screening processes. Document evidence of rehab admission criteria within established timeframe.
• Perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
• Provide information to patient/family/caregiver related to inpatient rehab and program features. Communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
• Obtain information relating to patient resources and healthcare insurance benefits.
• Demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.

What qualifications you will need:

• Graduate of an accredited School of Nursing or Physical Therapy School
• Current State of Florida Registered RN License, or
• Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provisions) or
• Physical Therapist License
• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Speech Therapist, or Occupational Therapy (OT), or Registered Nurse, or Licensed Physical Therapist
• Masters Degree, or Associate Degree
• 5 years of experience preferred
