Therapist - Physical Therapist

Job Summary:

Provide comprehensive physical therapy services to patients in a skilled nursing facility, aimed at restoring mobility, improving function, and promoting overall well-being. Collaborate with healthcare teams to develop and implement individualized treatment plans.

Responsibilities:

• 
  Assess patients physical conditions and establish treatment goals based on clinical evaluation.
  
  
• 
  Develop, modify, and implement personalized therapy plans to address patient needs.
  
  
• 
  Administer therapeutic exercises, manual therapy, and modalities to improve mobility and manage pain.
  
  
• 
  Monitor patient progress, document findings, and adjust treatment plans accordingly.
  
  
• 
  Educate patients and their families on therapy exercises and injury prevention strategies.
  
  
• 
  Collaborate with physicians and other healthcare professionals to optimize patient care.
  
  

Qualifications:

• 
  Valid state license as a Physical Therapist.
  
  
• 
  Proven experience working within a skilled nursing facility preferred.
  
  
• 
  Strong knowledge of physical therapy modalities and techniques.
  
  
• 
  Excellent communication and interpersonal skills.
  
  
• 
  Ability to work independently and as part of a multidisciplinary team.
  
  
• 
  CPR certification is required.
