Description: The Case Manager RN is responsible for discharge planning. A valid RN license is required for immediate consideration. Experience in an acute care hospital is essential. Behavioral health experience does not qualify for this role. BLS certification is preferred, not required. Candidates must have strong skills in acute hospital care and care coordination. Familiarity with CMS guidelines and medical necessity determinations is necessary. Candidates will need to utilize established criteria for precertifications and prior authorizations. Weekend shifts are required, either every other weekend or four weekend days within a four-week schedule. No on-call requirements are present for this position. Collaboration with interdisciplinary teams, including physical therapy and social services, is expected. Candidates must be open to floating within case manager services to meet department needs.
