Overview
Physical Therapist — General, Bala Cynwyd, Pennsylvania. This travel assignment in home health care starts September 16, 2026 and runs for 12 weeks. Weekly pay ranges from $2,555 to $2,664 per week, with guaranteed 40 hours. Shift is 8:00am–4:30pm, Monday through Friday (5x8 days). Responsibilities
- Deliver skilled physical therapy services to home health patients per the plan of care, including evaluation, treatment, and discharge planning.
- Conduct Start of Care (SOC) assessments and ongoing revisits, documenting progress and updating the plan of care as needed.
- Implement therapeutic interventions (gait training, therapeutic exercise, balance and mobility training, functional activity practice) and monitor patient response.
- Document in EPIC and complete required OASIS forms, maintaining accurate, HIPAA-compliant records.
- Collaborate with the interdisciplinary team and caregivers to optimize goals and ensure patient safety in the home environment.

Requirements
- Active Pennsylvania Physical Therapist license (or eligible for licensure)
- Recent home health experience; EPIC EMR experience; OASIS knowledge
- Experience performing Start of Care and revisits
- Ability to work in patients’ homes and travel for a travel assignment; valid driver’s license and reliable transportation
- Degree from an accredited physical therapy program (DPT or equivalent)
