Local Travel Home Health Physical Therapist
The company is hiring a Local Travel Home Health Physical Therapist for a 13-week contract in Springfield, MO. The role involves patient evaluations, individualized treatment plans, OASIS documentation, and care coordination to improve mobility and independence. The position offers up to $2,400 per week based on experience.
Key Responsibilities:
- Conduct comprehensive patient evaluations in the home setting
- Develop individualized treatment plans to improve mobility, strength, and independence
- Perform start-of-care assessments, recertifications, and routine visits
- Complete accurate OASIS documentation and maintain regulatory compliance
- Serve as case manager coordinating care with physicians, nurses, and interdisciplinary teams
- Educate patients and caregivers on therapeutic exercises and safety precautions
- Monitor progress and modify treatment plans as needed
- Document all interactions and outcomes using the Matrix EMR
- Deliver compassionate, patient-centered care promoting functional independence
Qualifications:
- Doctorate or Master's degree in Physical Therapy from an accredited program
- Active Missouri Physical Therapist license
- Current CPR/BLS certification
- Home health experience, especially for travel assignments
- Strong experience with OASIS documentation, case management, start-of-care assessments, and recertifications
- Experience using the Matrix EMR system
- Excellent communication skills
- Strong organizational and time-management abilities
- Ability to work independently in a home health environment
- Knowledge of home health regulations and compliance
- Ability to develop and implement individualized treatment plans
- Commitment to delivering compassionate, patient-centered care
Preferred Skills:
- Current CPR/BLS certification
- Home health experience for travel assignments
- Experience with the Matrix EMR system
