Job Title: Hospice Admissions Nurse – Registered Nurse (RN) | Per Admission Pay

Company: Superior Hospice, part of the Home Health Care, Inc. family

Compensation: $200 – $250 per admission

Benefits: Full Benefits Package for Full-Time employees

Location: Greater Twin Cities Metro Area, 21-county service area (territories assigned based on coverage needs)

Schedule: Full-Time or Part-Time with 1 on-call shift per week (backup) and every 4th–7th weekend

What You’ll Do

• Travel to patient homes, assisted living, and senior care communities to complete hospice admissions.

• Conduct clinical eligibility assessments and initiate services.

• Educate patients and families on hospice philosophy, benefits, and expectations .

• Collaborate with physicians, intake coordinators, and the interdisciplinary team.

• Coordinate care transitions to ensure a smooth start of hospice services .

• Maintain accurate, timely documentation in the EMR.

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.

• Experience in hospice, oncology, palliative care, or high-acuity settings .

• Confidence discussing end-of-life care and hospice services with patients and families.

• Strong clinical assessment, communication, and decision-making skills.

• Comfortable working independently in a field-based role .

• Valid driver’s license and reliable transportation.
