As the Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain and other medical conditions. Based on this assessment and evaluation, you will determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well‑being, and evaluate progress. $10,000 Sign on Bonus being offered for a full‑time placement for external candidates. Will consider relocating candidates. This field‑based position will cover Nicholas and Fayette Counties. Responsibilities Evaluate the patient's functional status and physical therapy needs and assist the physician in developing the therapy plan of care. Provide services within the scope of practice as defined by state laws and in accordance with the plan of care, coordinating with other health‑care team members. Observe and report the patient's response to treatment and any changes in condition to the supervising nurse or physician. Treat patients to relieve pain, develop and restore function, or maintain maximum functioning using acceptable standards of practice. Instruct and inform the patient, family/caregivers, and health‑care team about the plan of care, functional limitations, and progress toward goals. Visit patients according to the plan of treatment, complete a progress note for each visit, and submit notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state guidelines. Report outcomes of evaluation, goals, and anticipated projected frequency of care. Required Qualifications Current Physical Therapy licensure in the state of practice. Current CPR certification. Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Pay and Benefits Pay Range: $77,400 – $171,000 annual total cash target pay. Per visit point: $44.65 – $98.65. Hourly rate: $37.21 – $82.21. Annual total cash compensation assumes full‑time employment (40 hours weekly) at full productivity and typically follows the range above. Total cash compensation includes earnings from per‑visit point pay and hourly pay and is based on factors such as local labor markets, education and work experience. Compensation may increase over time based on productivity and performance. The role receives two types of compensation depending on the work performed. For visit activities, pay is based on per‑visit point rate and productivity points. For non‑visit activities such as orientation, the hourly rate applies.
