The RN works in both clinical and community settings, meeting clients where they are to improve engagement and reduce transportation barriers, with services delivered in healthcare facilities, community locations, homes, and other approved settings throughout Orange and Lawrence Counties. Qualified candidates must possess a valid driver's license, reliable transportation, the ability to travel throughout the service area, and strong communication, assessment, care coordination, and patient education skills. Experience in OB, maternal-child health, labor and delivery, postpartum, women's health, pediatrics, community health, community outreach, care coordination, patient navigation, and working with vulnerable or underserved populations is preferred, along with knowledge of maternal health resources, community programs, and social service systems.Shifts will primarily occur Monday-Friday, 8-5pm range with occasional evenings or weekends for outreach, community events, and supporting parent/community education.This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS.
