Job Title RN Case Manager / Nursing Supervisor Service Area Cincinnati, OH and surrounding areas in Hamilton County and adjacent counties. Schedule and Employment Type Part‑time or PRN. Travel up to 60 minutes for PRN/Part‑time (21‑31 hrs/wk) and up to 2 hours for full‑time (32+ hrs). Flexible, customizable schedule. No overnight or on‑call requirements. Responsibilities Perform client assessments and initiate appropriate care actions. Manage and supervise a multidisciplinary team of RN, LPN, HHA, and Personal Care Aides. Document, communicate, and coordinate patient status changes with the care team. Ensure compliance with safety and infection control protocols. Travel as required to surrounding counties. Maintain accurate OASIS and non‑OASIS assessments. Qualifications Graduate of an accredited School of Nursing. Active RN license in Ohio. Minimum 1 year of RN experience and 1 year of case‑management/home‑health experience. CPR and First Aide certification (or ability to obtain). Experience supervising RN, LPN, and aides; wound care, lab draws, OASIS assessment preferred. Strong communication, attendance, and confidentiality skills. Reliable transportation and valid driver’s license. Compensation Hourly Wage: $35–$40 per hour. Full‑time hourly or salary options. Benefits Round‑trip mileage reimbursement.
