RN Field Care Manager - Medicaid LTSS & Care Coordination

Molina Healthcare is seeking a Texas-licensed RN to join as a Field Care Manager in Fort Worth to assess members for services and support care planning. Preference for candidates with Medicaid/MCO experience; mileage is reimbursed and travel within 30–45 minutes of the coverage area is expected. The role focuses on coordinating LTSS, validating needs, and working with a multidisciplinary team to improve outcomes while controlling costs.
