Bilingual Complex Care Case Manager - Community Health Advocate

Marin Community Clinics seeks an Associate Case Manager to enhance health outcomes for patients with complex needs. You will support 30–40 patients, facilitate care plans, and coordinate with various healthcare providers. Ideal candidates will have an MA, CHW, or a Bachelor's in SW, and experience in complex care management. Bilingual in Spanish is essential.
