If you have a heart for helping others, enjoy meaningful one-on-one relationships, and want a flexible opportunity where your work truly matters, we'd love to meet you!What You'll Do:Complete psychosocial assessments for hospice patients and families.Identify emotional, social, financial, and practical needs affecting patient care.Provide emotional support, counseling, and guidance to patients and caregivers.Assist families with community resources, advance care planning, and end-of-life concerns.Support patients and families experiencing anticipatory grief, loss, and difficult transitions.Collaborate with nurses, chaplains, physicians, and other members of the interdisciplinary team.Participate in interdisciplinary team (IDT) meetings as needed.Advocate for patient needs, preferences, and dignity.Assist with crisis intervention and family support when appropriate.Complete accurate and timely documentation in the EMR system.What We're Looking For:Bachelor's or Master's degree in Social Work from an accredited program.Current Texas Social Work License (LBSW, LMSW, or LCSW).Previous hospice, home health, healthcare, or social work experience preferred.Hospice Social Work experience is a plus!Strong communication, assessment, and interpersonal skills.Compassionate and comfortable supporting patients and families during difficult situations.Ability to work independently while collaborating with an interdisciplinary team.Strong organizational and documentation skillsValid driver's license, reliable transportation, and current auto insurance.Ability to travel throughout Greenville and surrounding communities.What We Offer:Flexible PRN Scheduling.Competitive Compensation.Mileage Reimbursement.Supportive Leadership and Interdisciplinary Team.Meaningful One-on-One Patient and Family Relationships.Opportunities for Additional Hours as Available.A Team That Values Compassion, Communication, and Collaboration.Join a Best of Texas 2026 Award-Winning Hospice!Concho Hearts Hospice is proud to be recognized as a Best of Texas 2026 winner!We're locally owned and operated, and our mission is to provide compassionate care wherever our patients call home.
