Registered Dietitian (RD)

Duties/ Responsibilities:

• Complete nutritional assessments for new admissions and current residents as required.
• Monitor residents’ weight, meal intake, hydration, laboratory values, and overall nutritional status.
• Identify and address risks for malnutrition, unintended weight loss/gain, dehydration, and inadequate intake.
• Develop and monitor individualized nutrition care plans and interventions.
• Make recommendations for therapeutic diets, supplements, fortified foods, snacks, and enteral nutrition as appropriate.
• Participate in care plan meetings, interdisciplinary team meetings, and the MDS process as applicable.
• Collaborate with nursing, physicians, dietary staff, therapy, and other interdisciplinary team members.
• Provide nutrition education to residents, families, and facility staff.
• Monitor the effectiveness of nutritional interventions and update recommendations as needed.
• Maintain accurate and timely documentation in the resident medical record.
• Review and address nutrition-related concerns, including food allergies, diet preferences, swallowing/texture needs, and special dietary requirements.
• Participate in QAPI and other quality improvement activities related to nutrition and resident outcomes.
• Support compliance with applicable federal and California regulations, professional standards, and facility policies.
• Maintain resident confidentiality and protect personal health information.
• Assist with regulatory surveys and provide nutrition-related documentation when requested.
• Perform other related duties as assigned by the Administrator or Director of Nursing

Required Skills/Abilities:

• Strong knowledge of clinical nutrition and medical nutrition therapy.
• Knowledge of SNF regulations, MDS, and care planning.
• Strong assessment, documentation, and organizational skills.
• Excellent communication and teamwork skills.
• Ability to develop individualized nutrition plans and monitor outcomes.
• Ability to work independently and with the interdisciplinary team.
• Proficiency with electronic medical records and basic computer systems.
• Commitment to resident-centered care and confidentiality.

Education and Experience:

Any combination of experience and education that would be likely to provide the required knowledge, skills, and abilities could be qualified, as determined by the district. A typical way to obtain knowledge, skills, and abilities is:

• Bachelor’s degree or higher in Dietetics, Nutrition, or a related field.
• California licensure/registration, as applicable.
• SNF, long-term care, or healthcare experience preferred

Physical Requirements:

• Ability to sit, stand, and walk throughout the facility.
• Ability to bend, reach, and access resident rooms and common areas.
• Ability to lift and/or carry up to 25 pounds as needed to perform job duties.
• Ability to use a computer and electronic medical record system.

Salary:

$40.00–$60.00 per hour, depending on experience, qualifications, and position status.

Work Schedule:

• Part-Time: Schedule based on facility and resident needs.
• May require flexibility to work evenings, weekends, or holidays as needed to support residents and facility operations.
