Occupational / Physical / Speech Therapist - In House

We’re seeking a compassionate and skilled Therapist to join our care team. In this role, you’ll help residents regain independence, enhance daily living skills, and achieve their highest possible quality of life. You’ll work closely with nursing, medical staff, and other departments to ensure therapy services are effective, resident-centered, and compliant with all state and federal guidelines.

*Key Responsibilities*

* Provide individualized therapy evaluations and treatments for residents.
* Participate in care planning, MDS assessments, and discharge planning.
* Maintain accurate and timely documentation, including treatment notes and progress updates.
* Collaborate with interdisciplinary team members to coordinate therapy services.
* Educate residents, families, and staff on therapy techniques and functional goals.
* Ensure therapy services align with safety standards, infection control, and facility policies.
* Support residents in setting goals and taking an active role in their treatment.

*What We’re Looking For*

* Licensed Therapist (OT, PT, ST) in the state of Wisconsin.
* Strong communication, teamwork, and organizational skills.
* Commitment to resident dignity, safety, and high-quality care.

Pay: $55.00 - $65.00 per hour

Expected hours: 16.0 – 24.0 per week
