Job Summary The hospice registered nurse plans, organizes, and directs home care services with a focus on community health education and experience. You will work closely with individuals and families within their homes and communities, utilizing available resources to meet their unique needs. Duties & Responsibilities Patient Care Develop and initiate personalized care plans in partnership with patients, representatives, and caregivers. Implement preventative and rehabilitative nursing procedures as ordered by physicians. Provide counseling and education to patients, caregivers, and families. Prepare clinical notes and maintain effective communication with the healthcare team. Identify discharge planning needs and implement appropriate measures. Communication Update primary physicians and communicate any changes in patients' conditions. Coordinate care plans with community health-related individuals. Additional Duties Participate in quality assessment and performance improvement programs. Conduct in-service training and provide supervision to home health aides. Ensure availability of necessary equipment and services. Instruct, supervise, and evaluate home health aide care. Supervise Licenced Practical Nurses (LPNs)/Licenced Vocational Nurses (LVNs) and/or Home Health Aides (HHAs)/Certified Nursing Assistants (CNAs), as directed. Job Requirements Graduated from an approved school of nursing and licensed in the state of practice. Minimum of two years of experience, with preference given to those with public health or home care nursing experience. Current CPR certification. Licensed driver with a reliable and insured automobile. Self-directed and able to work with minimal supervision. Excellent observation, problem-solving, and communication skills. Ability to organize and prioritize workload independently. Management experience not required. Physical ability to perform required nursing skills.
