Certified Diabetes Educator

Overview The incumbent will develop, manage, and coordinate all aspects of the outpatient diabetes program and is responsible for outreach to physician practices as a referral source for the diabetes program. Minimum and Preferred Qualifications Education, Certification, and/or Licensure: Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education. Experience: One (1) year experience in health and fitness. Preferred Education, Certification, and/or Licensure: ASCM, Clinical Exercise Physiology Certified. Core Duties and Responsibilities Identifies education needs of the community and those with diabetes. Attends diabetes advisory committee meetings as assigned. Performs patient assessment, delivers education and evaluates patient’s obtainment of goals. Assists in maintaining the American Diabetes Association certification as applicable. Participates in community outreach and educational support. Advocates on behalf of the patient and families. Communicates with primary care providers, members of the multi‑disciplinary team regarding relevant questions, concerns, and patient outcomes. Participates in performance improvement activities to assure quality care. Provides administrative support and coordinates the management and operations of the Diabetic Institute. Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program. Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials. Physical Requirements Ability to lift 25 pounds. Ability to push/pull 100 pounds. Ability to stand for several hours per day. Working Environment Wellness Center. Skills and Abilities Computer skills, particularly Excel and Word.
